Grant Descriptions
Effectiveness of obstructive sleep apnea (OSA) home monitoring technologies in patients with Heart Failure
The purpose of this research is to broaden association member nurse involvement in clinical research testing the effectiveness of obstructive sleep apnea (OSA) home monitoring technologies in patients with heart failure.
Background: OSA has a known prevalence of 40-60% in patients with heart failure, and is associated with a variety of comorbid complications, including hypertension, arrhythmias, stroke, MI recurrence, and insulin resistance. Mounting evidence suggests that treatment of OSA with positive airway pressure decreases the incidence and severity of these co-morbid conditions in affected individuals. Likewise, patients with heart failure who are effectively treated for OSA frequently report improvements in depressive symptoms, quality of life, daytime sleepiness, and daytime functioning.
Sleep questionnaires alone have been shown to be inadequate to fully assess OSA in patients with heart failure. In addition, polysomnography (PSG, in-lab sleep study) is both expensive and inconvenient. This inconvenience is characterized by patient reluctance to go for overnight studies, lack of PSG lab accessibility to some community-based heart failure providers, and scheduling delays.
It is estimated that less than 5% of patients with heart failure are currently being tested for OSA. The recent national approval by CMS for home sleep testing (HST) offers a potentially valid, reliable, and economic solution.
Proposals: Research must be conducted by an association nurse investigator, be hypotheses-driven, and must include Nexan OSA home monitoring technology.
Two individual awards of up to $15,000 in financial support will be granted to qualifying AAHFN members with approved proposals. The OSA home-testing equipment will be provided by Nexan at no charge to the investigator.
