AAHFN Group Registration

AAHFN is pleased to offer a special discount to groups of three (3) or more attendees.
Additionally, groups with six (6) or more will receive one (1) complimentary registration.

Step 1:	Complete the Group Registration Cover Sheet. Click here to download a copy of the cover sheet. The cover sheet can be faxed ahead of time or with the registration forms.
Step 2:	Each attendee is required to submit a completed registration form. Click here to download a copy of the registration form. Online registration is currently not available for group registration. While we prefer to receive the registration forms all at once, we understand that this is not always possible. As long as we have your cover sheet on file, we will be able to track your group.
Step 3:	Fax the cover sheet and registration forms to - Lisa Dougherty, Registration Manager, at 856-439-0525

Notes: Each attendee will be responsible for picking up their own registration materials at the meeting. Should group attendance fall below three (3) attendees, current registrations fees will apply. Industry registrants and exhibitors are not eligible for the group registration discount.
