Heart Failure Programs: Everything You Wanted to Know but Didn't Know Where to Ask.

AAHFN is offering this exclusive workshop in conjunction with the Annual Meeting.
This program results from the interests and desires of our members as well as from the gap in educational resources currently available for clinicians on this subject.
The purpose of this workshop is to provide attendees with evidence-based information on the essentials of heart failure disease management programs (HFDMP), including pathophysiology, pharmaceutical and non-pharmaceutical interventions, space and personnel requirements, financial issues, and marketing ideas.
All attendees will receive tools and information necessary for implementing a heart failure program at a local level or for enhancing an existing heart failure program, including a workbook with examples of business plans, prospectus, policies, procedures, patient education, and evaluation/outcome measurement tools.
Educational content will be presented by heart failure nurse experts with demonstrated experience in implementing and managing disease management programs, who will share their knowledge and advice with participants.
Objectives
At the end of the program, the participant will be able to:
- Identify the key space, personnel and supply requisite for starting a heart failure program.
- Discuss three elements that should be included in a heart failure program proforma.
- Discuss financial and clinical outcome measures for a heart failure program.
